How do I schedule an appointment?

What should I bring to my first visit?


Bring the following if it pertains to you:

• Your current driver’s License or other government issued photo I. D.
• Your current Insurance card and applicable co-payment
• Relevant medical and immunization records
• A list of all the medications you’re taking including dietary supplements
• Your Office Visit co-pay. This will be indicated on your Insurance ID card
• A completed paper copy of your New Patient Registration Packet.

Plan to arrive at least 30 minutes before your first appointment to complete paper work. Your appointment time indicates the time your doctor is scheduled to see you. You’ll need to allow extra time to park, check in with our Front Desk, and complete any necessary forms.

Download and complete the New Patient Registration Packet. When you schedule an appointment, we will send you a link to your New Patient Registration Packet. It’s important to complete the New Patient Registration Packet before you arrive for your first appointment to save wait time. It includes questions about your current health status, family history, medications, plus important information we need to set up your account.

Send us your previous medical records. As soon as possible, arrange to have any previous medical records available for your new PCP to review. You may bring copies with you, or have your previous provider send them directly to Eastway Medical Clinic and Urgent Care. (Note: You’ll need to authorize your previous doctor to release your medical records to us. Please download and complete the record release form and send it to your previous doctor’s office.)

What should I do if I am running late or cannot keep my scheduled appointment?


Contact our office as soon as possible. We will do our best to accommodate your needs and will try to reschedule your appointment for a time that fits your schedule if needed.

What should I do if I have an urgent medical problem?


In the event of an emergency after office hours, call our office. A physician is always on call during the hours the office is closed. Leave a message if you do not get immediate reply and we will return your call as soon as possible.

If you believe it is a life-threatening problem, call 911 immediately. If you go to the Emergency Department, you should take vital information including your primary physician’s name, any medication you are taking, and a list of all known allergies.

Requests for medications or medication refills should be phoned into the office on weekdays. We ask that you do not wait until your prescription is completely gone before calling for a refill. Please allow 48 hours for the processing of your prescription request. Controlled substances may not be refilled without an office visit.

May I request copies of my medical records?


Yes. Please make this request at our office. You may be charged a fee for this service.

Your doctor’s office will give you the necessary forms to complete for a transfer of your medical records. You may be charged a fee for this service.
